A blog on the Alliance Self Management Network Scotland website takes a look at the work of the Forth Valley Anticipatory Care Planning team and their approach to self management support.
The article reviews their strength based approach that links people into local sources of support, and seeks to reconnect them with their wellness and resilience, rather than their ill health.
The role of the the Anticipatory Care Planning team is described as providing an important bridge between health and social care, working collaboratively with social work teams in the area. Their role is not to try and prevent admissions to hospital, although this can be an outcome of their support, but are there to work with people for as long as is needed to make sure the person and their family feels ready to cope.
