    New self care app for people with pain, anxiety or breathing difficulties

    In partnership with local CCGs, Kirklees Council have just launched a new interactive app – ‘My Health Tools’ – to support self-care and behaviour change. It is being piloted using three long-term conditions: pain, anxiety and breathing difficulties.

    My Health Tools has been designed by and for local people, and will support them to better manage their needs, monitor their condition and help them to feel more in control.  It provides access to a wealth of trustworthy information and resources giving people more choice over their care and the opportunity to access extra support.

    My Health Tools aims to help people make the changes they want, and improve their lives whilst living with an LTC. It can help people to:

    • Feel in control of their lives while living with their health condition
    • Increase their confidence to better manage their health condition
    • Be inspired with many new ideas about how to best manage their health condition
