The Health Foundation have published a new ‘Practical Guide to Self Management Support‘.
The resource provides an overview of self-management support and the key components for effective implementation. It aims to be useful both for those starting their self-management support journey and those building on and improving the support that they already provide.
The guide explains what self-management support is and why it is important. It looks at various aspects of putting self-management into practice, including planning and commissioning, and measurement and evaluation.
The guide discusses the different elements of building the knowledge, skills and confidence that support people to self-manage, including patient information websites, education courses and recovery colleges.
It also contains suggestions for further reading and case studies of self-management support in practice.
