This is the booking form for the PiF Annual Conference 2012. Please read the information below before you complete the form.
There are a number of ways that you can pay for your delegate place, but you must complete the booking form regardless of which method you want to use.
If you would like to pay by cheque or BACS (electronic bank transfer), complete the form below and submit it. We will automatically send you an invoice when we receive your booking.
If you would like to pay by credit or debit card, complete the booking form and press the submit button. You will then be given the opportunity to go to a secure payment web site (Pay Pal).
Concession rates are for PiF members only. If you are not a PiF member the full ticket rate applies.
If you come with one or more colleagues from your organisation you will benefit from a reduced rate of 15% on the total cost of your booking (any other discounted rates do not apply). If there is more than one person coming from your organisation please contact Nicole Naylor, PiF's administrator, now and do not complete the booking form.

An invoice will automatically be sent to the name and address above. If you would like the invoice to be sent to your Finance Department, or if you are providing a purchase order number, please provide details below:

£204 +VAT (£244.80) for PiF members £284 +VAT (£340.80) for non-members from the voluntary and academic sectors and from the NHS £350 + VAT (£420) for non-members from commercial organisations The fee includes lunch, refreshments and a copy of the conference CD.

You must pay for your place before the event, or we will not be able to admit you on the day.
A refund, less a 20% administration fee, will be made if cancellations are received, in writing, at least 4 weeks before the conference. We regret that any cancellation after this time cannot be refunded, and that refunds for failure to attend the conference cannot be made, but substitute delegates are welcome at any time.
You will receive confirmation of receipt of your booking form within 10 days. Further details regarding the event will be sent to you once payment is received. Please contact us if you have not received confirmation 10 days after submitting your booking form.
On receipt of your delegate fee you will receive details of accommodation.
