The Patient Resource of the Year at the BMA Patient Information Awards was awarded to Rotherham NHS Foundation Trust for their Patient Information Diary. The diary is a communication aid between patients with dementia and their carers and healthcare workers. It aims to provide a basic awareness to healthcare workers of the needs of patients with dementia.
The diary contains sections on personal history, communication, daily routine and respect and dignity. To see a copy of the diary click here.
